5. Your location (Max 3) (Madison – West, Madison – Near West, Madison – Central, Madison – East, Madison – North, Madison – South, Middleton, McFarland, Verona, Sun Prairie, Stoughton, Oregon, Fort Atkinson)
6. Address as you would like it to appear on the website (state and Zip code not necessary) If you wouldn’t like your address published, please replace with a general area, neighborhood, or street.
